The Lake County Sheriff’s Office is investigating a fatal head-on collision that left one man dead and another injured early Wednesday morning, September 3, 2025, in Ingleside.
Early Morning Crash
At approximately 5:00 a.m., sheriff’s deputies were dispatched to Route 134 east of North Catherine Street after receiving a 911 call from one of the drivers involved, who requested medical assistance. When deputies arrived on scene, they discovered two vehicles—both heavily damaged—blocking the roadway.
Detectives determined that a Honda sedan, driven by an unidentified male, had been traveling westbound on Route 134 when, for reasons still unknown, it crossed over the center line into the eastbound lanes. The Honda then collided head-on with a GMC work truck driven by a 44-year-old Johnsburg man.
The impact proved fatal for the Honda’s driver, who sustained critical injuries and was pronounced deceased at the scene. Investigators reported that an open bottle of whiskey was found in his lap, raising concerns that alcohol may have been a contributing factor in the crash.
The GMC driver survived, suffering non-life-threatening injuries. He was transported to a local hospital for treatment and has since been cooperating with investigators.
Stolen Vehicle Link
Authorities later confirmed that the Honda involved in the collision had been reported stolen the previous day from a gym parking lot in Fox Lake. Investigators are now working to determine how the deceased driver came into possession of the vehicle and whether he had any connection to the theft.
Investigation Ongoing
The Lake County Coroner’s Office responded to the crash site. An autopsy is scheduled to confirm the driver’s identity and determine the official cause of death. Toxicology results are also pending and are expected to shed light on whether impairment contributed to the crash.
Meanwhile, the Lake County Sheriff’s Technical Crash Investigations Team continues to reconstruct the events leading up to the collision. Investigators are examining roadway evidence, vehicle data, and any available surveillance footage from nearby locations.

The Ingleside crash has left local residents rattled, as Route 134 is a well-traveled corridor for early morning commuters. For many, the incident serves as a sobering reminder of how quickly lives can be changed—or lost—on the road.
